    • Acyclovir/Valacyclovir/Famciclovir — TK-dependent, HSV/VZV
    • Ganciclovir/Valganciclovir — CMV, bone marrow toxic
    • Foscarnet — direct polymerase, for resistant (severe electrolyte)
    • Cidofovir — broad polymerase, severe nephrotoxic
    • Letermovir — CMV terminase, no marrow toxic, post-HSCT prophylaxis
    • Maribavir — UL97 kinase, refractory CMV, dysgeusia
  • Influenza: oseltamivir (NA inhibitor, < 48 hr), baloxavir (single dose)
  • RSV: nirsevimab (mAb, universal infants 2024), maternal vaccine alt
  • COVID-19: Paxlovid (Mpro inhibitor, drug interactions!), remdesivir IV, molnupiravir alt
  • Mpox: tecovirimat (STOMP NEGATIVE 2024)
  • Adenovirus: cidofovir / brincidofovir
  • JC PML, BK: immune reconstitution; no specific drug
  • Pregnancy: oseltamivir + acyclovir safe; molnupiravir contraindicated
