  • Modified Duke: 2 major OR 1 major + 3 minor OR 5 minor
  • Empirical: Vanco + Ceftri (native) / Vanco + Cefepime (acute) / Vanco + Gent + Cefepime (early prosthetic)
  • Duration: 4-6 wk IV (4 wk S. viridans native, 6 wk S. aureus, 6 wk prosthetic + gent + rif)
  • Surgery indications: HF, uncontrolled infection, prevent embolism, prosthetic dehiscence, fungal IE
  • Skin signs: Janeway (painless palm/sole), Osler (painful pulp), Roth spots (retinal)
  • S. gallolyticus (S. bovis): colon cancer screen
  • Prophylaxis: high-risk only, dental + respiratory only, amoxicillin 2g PO
  • 22E: POET oral switch trial, PET-CT, daptomycin for MRSA
