300 Ch 299. Rheumatic Fever and Rheumatic Heart Disease

Acute rheumatic fever (ARF) 是 Group A Strep (GAS) 咽喉炎後 2-4 週的全身性自體免疫反應，影響心、關節、皮膚、神經、皮下組織；Jones criteria (2015 AHA modified) 為診斷標準，包含 major + minor + 確認 GAS infection；rheumatic heart disease (RHD) 是慢性後遺症 → 心瓣膜病變（MS 最常見、MR、AR、AS 各有特徵）；全球仍是兒童心血管疾病主因（每年 30-40 萬死亡，多在 sub-Saharan Africa、南亞、太平洋島）；primary prevention：penicillin 治療 GAS pharyngitis；secondary prevention：long-term penicillin prophylaxis 預防復發 (BPG 1.2 MU IM q3-4 wk)；echo-confirmed RHD in asymptomatic = 推薦 secondary prophylaxis (RHD action plan 2024)；handheld echo screening + AI-assisted diagnosis 是 2020s 新策略；台灣 ARF / RHD 已大幅減少 但仍有偶發案例。
