281 Ch 280. Diseases of the Aorta (Aneurysm + Dissection)

主動脈疾病 三大族群：(1) aneurysm（thoracic TAA / abdominal AAA）；(2) acute aortic syndromes (AAS) = dissection + intramural hematoma (IMH) + penetrating ulcer (PAU) + ruptured aneurysm；(3) inflammatory / infectious / genetic (Marfan, Loeys-Dietz, vEDS, Takayasu, GCA, mycotic)；AAA 篩檢：65-75 男性吸菸者一次 US（USPSTF Class B）；AAA 修補 ≥ 5.5 cm ♂ / 5.0 cm ♀ 或 ≥ 0.5 cm/年；TAA 修補 ≥ 5.5 cm（Marfan ≥ 5.0；Loeys-Dietz ≥ 4.0）；Type A dissection = emergency surgery；Type B = medical first (esmolol → nicardipine, SBP < 120); complicated → TEVAR；IRAD registry 數據是基石。
