140.2.0.1 必背 — Uncomplicated Cystitis 1st line

1. Nitrofurantoin × 5 d
2. Fosfomycin × 1 dose
3. TMP-SMX × 3 d (local resistance < 20%)

(Fluoroquinolone NOT first-line for uncomplicated)

140.2.0.2 必背 — Pyelonephritis

• Outpatient: cipro × 7 d
• Inpatient: ceftriaxone IV → de-escalate
• Severe: pip-tazo or carbapenem (ESBL risk)

140.2.0.3 必背 — Asymptomatic Bacteriuria

• Treat ONLY: pregnant, pre-urologic procedure, < 1 yr post-renal transplant
• Don’t treat: 老人, catheter, DM 無症狀 (overtreatment harm)

• Young female honeymoon: S. saprophyticus
• Stone / urolithiasis: Proteus (urease, struvite stone)
• Hospital / catheter: Pseudomonas, ESBL E. coli
• Candida: catheter, DM, immunocompromised
